Alameda Health System (AHS) has partnered with Cardea Health to provide wrap-around services and permanent supportive housing to its most vulnerable patients.
Unhoused people who find themselves in AHS emergency rooms sometimes need ongoing supportive medical care after their acute needs are met. But instead, they often end up back on the street.
OakDays is a former motel that now offers housing with around-the-clock medical care to its residents in Oakland. AHS has partnered with Cardea Health, one of the nonprofits that runs OakDays, to house and care for patients long term.
